Trying to sweep a standard surface, not working as expected

Hi all

I am using NSQ in Zemax, and trying to set up a swept standard surface, as seen on the attached figure:

However, I am not getting the sweep I want. I actually do not get any sweep. As said in this post, 

The explanation for using a swept surface is a little bit lacking in the Zemax manual. From this post, I've gathered that there is something wrong with my coordinates, but I’m not sure how to fix it. Any help on this would be of great help. I’ve been playing with numbers for a few hours with no success. 

What I ultimately going for is a 360 degree sweep around the z-axis of the standard surface to create some semblance of a custom annular mirror that I can then optimize,
