First, can someone confirm that ht Through Focus MTF tool works by modifying the thickness of the second-to-last surface (when the last surface is the image plane) ?
If so, would it be possible to specify which surface thickness to vary ?

I would assume so as well, but I can’t confirm 100%.
Regarding your second point, I guess you could do that with the ZOS-API, but have you considered using the Optimize..Slider feature in combination with a Spot Diagram? You can have a Slider on any of the surfaces thickness, and it will update the Spot Diagram accordingly. The range of the slider can be tuned, but then it seem to always be 50 steps over the range.
