How to convert CodeV Decenter type "Global Coordinates' into Zemax

I have a surface “OBJ_EP’’ in Code V. Its thickness is 7.049 and the decenter of it is below

 

What I did in Zemax: create 3 coordinates breaks ahead of the surface as image below. The first one is coordinate return Orientation XYZ to the global reference surface “ILL_IMS” as CodeV. The second is YZ shift. The final one is tilts (I am aware of that the tilts sign is opposite b/w CodeV and Zemax)

However the convert result does not look right. Can anyone help? thanks
